Ambulance Membership

Ambulance membership money is used to purchase vehicles and equipment for the ambulance.  In order to provide a reliable service to the public, the Johnsonburg Fire Department finds it necessary to replace an ambulance every 4 years.  The ambulance purchased in 2007 came at a cost of $135,000.00.  There is another $40,000.00 worth of equipment an supplies on each ambulance.  Your support is important to keep this valuable service operating.

2010 / 2011 Ambulance Membership application information

To apply for membership you must fill out and sign the Johnsonburg Fire Department Ambulance membership card.  A signature is required due to insurance company restrictions, and if the card is not signed it will be returned to you and your membership will not be processed.

For each ambulance call your Insurance carrier, Medicare, or Medical Assistance is billed.  Most of these insurers don't pay the bill in full, but pay an amount that they feel is fair.  Membership covers the gap between what your insurance covers and any remaining balance that may be passed on to you.

The stub on the right side of the card is to be removed and kept by you as your membership card.  No other membership card will be sent.  Please keep your card in a safe place.
